Hospital in the
Home
Hospital in the Home (HITH) is a program that enables
people with specific illnesses to have part or all of their treatment at home
rather than in hospital.
People can use the HITH service if:
- Your condition is stable
- You feel comfortable with the idea of treatment at
home and have support if required
- You have an understanding of the treatment plan so
you can respond to changes in your condition at home
- You agree and follow the advice of the nurses and
doctors who are responsible for your care and treatment at home
- Your home environment is safe for visiting staff
Once on this program:
- If you are not getting better or your condition
worsens, you will return to the Emergency Department for assessment and
possible admission
- You will receive your first treatment in the
hospital. This is usually in the Emergency Department
- You will have your own blue HITH folder. You will
need to take the folder to all appointments, tests and if returning to
the Hospital
- A time will be worked out for your HITH visit and
visits will occur approximately the same time each day
Your rights:
- Access high quality care and treatment
- Receive care with dignity and in privacy
- Receive clear information and be able to seek
clarification about all aspects of your condition, care and treatment
- Discontinue the service for any reason
- Complain about the services without fear of
reprisal
Your responsibilities:
- Tell health care workers of any change in your
condition or circumstances so they can make appropriate decisions
regarding your health needs
- To provide a safe environment for health care
professionals during visits. ie. Keep pets under control, refrain from
smoking and provide adequate lighting/access at night
- To ensure the safe keeping of your folder,
medications and equipment during your treatment.
