Heart Failure Rehabilitation Program

What is heart failure rehabilitation?

Heart failure rehabilitation is an out patient program for people with stable heart failure.  It involves participation in exercise classes, education and support sessions, aiming to improve physical function and self-management of your medical condition.

 

Who attends?

Any person with stable heart failure can be referred to attend rehabilitation.  Referrals can be made by the hospital if you have been recently admitted, or from your local doctor.

 

Exercise sessions are tailored to meet your individual requirements and goals.  Education sessions are designed to be informative and interactive.  A support group will be available on the Friday over lunch.

·         Providing you with the confidence to continue exercising independently upon completion of the program

 

What is involved with this program?

You will need to attend supervised exercise sessions each Tuesday and Friday for 8 weeks. Each Tuesday will include an education session to improve your ability to self manage your medical condition.  Your exercise program is specifically designed to maximise your ability to complete day to day activities easier.

 

When is the program held?

Tuesdays 11:30am – 1.30pm

§         Exercise from 11:30am to 12:30pm, followed by lunch and education session.

Fridays 11:30am – 1:30pm

§         Exercise from 11:30am to 12:30pm, followed by lunch and support group.

Please bring your own lunch. Refreshments will be provided.

 

Transport

The use of private transport is encouraged.  If clients are unable to access private transport, information regarding alternative

arrangements can be provided.  If you have concerns regarding transport please direct them to the CRC Coordinator or HARP Coordinator.

 

Notification of Non-attendance

HARP must be notified if a client is unable to attend. Where possible, advance notice of holidays or other appointments should be given.

Hours of operation:  830am – 5.00 pm

Monday to Friday. Closed Public Holidays.
