Applications

For all research, full ethical review is required and you will be required to complete and submit a full application to the LRH HREC for review and approval.  Ethical approval can be granted for up to 5 years.

 

Applications should include LRH application module, the National Ethics Application Form (NEAF) along with the Victorian Module in line with the Department of Health single site research process. Note that the Department of Health Common Application Form is also still accepted.

If you require hospital resources to conduct your research project, please ensure you submit a Statement from Head of Department and Divisional Director Declaration form for each hospital department involved.

Please note that a precondition of the HREC is that the scientific merit of the research proposal has been guaranteed.  Approval of the scientific merit of the project through research approval can be evidenced by obtaining:

 

a) Ethics approval by an established Tertiary Education Facility (University) HREC

OR

b) Approval by a nominated LRH HREC representative from Monash University School of Rural Health

A copy of the approval letter of scientific merit or HREC approval from a tertiary institution is required to accompany your application.

 

 

Submitting your application

An original single-sided plus one (1) double-sided stapled copies of the completed application form and attachments should be forwarded to the HREC Secretary at least four weeks before the ethics committee meeting at which you wish it to be considered.  See meeting and deadline schedule.  The copies should be collated and paginated (eg Page 1 of 50).

Notification Form

The Ethics Committee requires notification in writing to the HREC Secretary of the project commencement date or if the research is discontinued before the expected date of completion.

Interim or Annual Reporting

The Ethics Committee is required to monitor all approved research in accordance with the National Statement on Ethical Conduct in Human Research (2007). Part of the monitoring process includes the review of progress reports.

 

Researchers are required to submit a progress report to the HREC for each research project approved by the Ethics Committee. Reports are usually due on the anniversary of approval, but occasionally they are requested at a more frequent interval.

 

Ethics approval is ongoing but will lapse if the progress report is not received by the anniversary of the approval date, or, is not deemed satisfactory by the Ethics Committee.  If the Committee is not satisfied with the progress report, researchers will be informed about subsequent action.

Amendments/Modifications

From time to time amendments are required to research projects. These are usually initiated by the researchers or by external study sponsors. Request for protocol amendments must be submitted on the Latrobe Regional Hospital Amendment Request Form to the HREC Secretary, Latrobe Regional Hospital, PO Box 424, Traralgon, Vic, 3844.

Final Report

Reports are due on the completion of a project or if the research is discontinued before the expected date of completion.
