Referrals to inpatient sub-acute care can be made by medical professionals such as doctors and GPs.
All referrals to inpatient sub-acute units require a form to be completed. You can download the referral form here. The form is in PDF format and may be completed online or printed. All referral forms are to be completed and faxed back to the appropriate inpatient unit. Contact details are below:

Before making a referral to inpatient sub-acute care at LRH, consider checking the patient meets the admission criteria for inpatient sub-acute care. The criteria can be found here. Some inpatient sub-acute units may have further criteria required for admission. Level 4 sub-acute: is a regional service that provides a full range of clinical services with specialist assessment and management across multiple clinical programs. It is supported by well-developed outpatient clinics. Level 3 sub-acute: is a sub-regional service that provides a dedicated program for a broad range of services and definitive care suitable for most sub-acute patients. It is supported by well-developed outpatient clinics. Level 2 sub-acute: is considered a local service that provides a single stream and/or restorative care service with the aim of maintaining function in patients with less complex needs. It is supported by core allied health and nursing staff. MDT: A multidisciplinary team (MDT) is a group of health care workers who are members of different disciplines each providing specific services to the patient. Often referred to as an interdisciplinary approach, it involves the successful and productive interaction of many stakeholders with expertise specific to a patient’s needs Medically Stable: When the acute phase of the illness has been managed and a documented plan has been identified for the next phase of care. For patients who reside outside of Latrobe City, consider referring to an inpatient subacute care unit closer to their home.
